The song was included on a 20-track demo compilation put together by Del Rey in March 2009, titled Demo March 2009. The original file of the song leaked on January 7, 2024.
Bellevue Hospital is a hospital in Manhattan, New York, closely associated with its historically poor treatment of mentally ill patients. This song heavily references Lana's time being institutionalized, a topic she mentions in "Fingertips" as well as in the title of her yet-to-be-released second poetry book, Behind the Iron Gates – Insights from an Institution. On the campus of Bellevue, a set of recognizable iron gates close off the front lawn of the hospital. Courtney Love, a close friend of Lana's, was once a patient at the hospital. She has frequently mentioned her (as well as Del Rey's) institutionalization at Bellevue in Instagram comments.[1]
